In the event that you will be returning 2017-18 Sanofi Pasteur Fluzone Quad MDV, Fluzone Quad PFS, Fluzone Quad SDV, Fluzone Intradermal Quad and/or Fluzone High-Dose influenza vaccine products, here are the necessary steps to complete the process.

For the 2017-18 season, up to 30% of each Sanofi Pasteur product presentation ordered direct from FFF and listed below are eligible for return for the full purchase price as a credit to be applied to account:

• Fluzone Quad - 5 mL MDV, 0.25 mL PFS, 0.5 mL PFS and 0.5 mL SDV
• Fluzone Intradermal Quad - 0.1 mL prefilled microinjection
• Fluzone High-Dose - 0.5 mL PFS

Full 10-dose vials and boxes of 10 prefilled syringes must be in their original unopened cartons. No partial returns will be accepted.

RETURN DATES

Fluzone Quad MDV, Fluzone Quad PFS, Fluzone Quad SDV, Fluzone Intradermal Quad and Fluzone High-Dose influenza vaccine returns will be accepted by FFF starting Monday, May 14, 2018 but no later than Friday, June 15, 2018.

INSTRUCTIONS FOR RETURN OF GOODS AUTHORIZATION
To ensure a successful return process you MUST submit and request for a Return Goods Authorization (RGA) number. Go to MyFluVaccine.com to complete the online RGA request form. Please follow and read the complete instructions here.

Important: Returns cannot be accepted without this RGA number!

SHIPPING

Customers are responsible for arranging and paying for shipping eligible return product to FFF Enterprises.

No refrigerated packaging is required, however all product must be properly packaged so that it will not be damaged during transit.

Sanofi Pasteur INFLUENZA VACCINE ONLY
This information applies to the Fluzone Quad MDV, Fluzone Quad PFS, Fluzone Quad SDV, Fluzone Intradermal Quad and Fluzone High-Dose for the 2017-18 season only.

Information and guidelines for return of all other brands of influenza vaccine for the 2017-18 season will be provided in other communications.

Please remember the deadline for returning Sanofi Pasteur influenza vaccine to FFF is Friday, June 15, 2018.
